
72-year-old man presenting with shortness of breath and cardiac angina. Diffuse crackles (Velcro rales) were found at auscultation. Clubbing of the fingers at clinical examination.
Peripheral lung biopsy showing honeycombing. The normal lung parenchyma is replaced by large fibrotic areas. Residual airspaces are lined by ciliated cells and proliferation of smooth muscle signs the long-standing pathology.
